The service
Client Form Portal provides software for insurance professionals and agencies: secure client forms (private, expiring links your clients use to submit requested details such as banking information for an insurance application), live view-only screen sharing that is never recorded, and an optional Chrome extension. We are not an insurance carrier, agency, broker or payment processor. Submitting a form does not authorize a payment or withdrawal, and we do not submit anything to carriers.
Your account
Give accurate information and keep your password confidential. You are responsible for activity under your account and workspace. Workspace owners control their team's seats; removing a seat ends that person's access immediately.
Acceptable use
Only request client information you are permitted to collect, for legitimate insurance-application purposes, and obtain any consent your clients require. Only share screens appropriate for the viewer. Delete submitted client details when you no longer need them. Do not use the service for unlawful collection, phishing, impersonating carriers, reverse engineering, overloading the service or bypassing plan limits.
Plans and usage limits
Free plan: screen sharing only, 15 sessions per month, up to 15 minutes each; sessions shorter than 40 seconds don't count and unused sessions don't roll over. Paid individual plans (per agent, per month, USD): Client Forms $20, Screen Sharing $20, Forms + Screen Sharing $30; paid sessions last up to 30 minutes. Agency Client Forms plans: Small Agency $79/month (5 seats), Growing Agency $199/month (15 seats), Agency $349/month (30 seats). Larger agencies and agency screen sharing are quoted on request.
Billing and renewal
Paid plans are billed monthly in advance in USD through Stripe and renew automatically each month until canceled. There is no annual billing and no free trial of paid plans. If a payment fails, your paid features may be paused or your plan moved to Free.
Cancellation
Cancel anytime from Billing in your account, which opens Stripe's secure billing page. After canceling, your plan will not renew.
Refunds
Payments are non-refundable. We don't provide refunds or credits for partial months, unused seats or unused features, except where required by law.
Client data and privacy
Our Privacy Notice explains how information is handled. Banking details are encrypted when stored and only the agent who sent the request can view them. You remain responsible for how you use client information once you view it.
Changes and availability
We may improve or change features and these terms. We'll post updates here and notify account holders of material changes by email or in the app. We aim for reliable service but can't guarantee it will be uninterrupted. We may suspend accounts that violate these terms or put clients or the service at risk.
